耳积水性疾病：梅尼埃病和鉴别RobertGU?RKOV，EikeKRAUSE

梅尼埃病是一种发病率在0.2%-0.5%的慢性病，其特征是发作性眩晕、波动性听力下降、耳鸣、耳闷胀感和进行性听前庭功能减退。年前，ProsperMeniere首次认为内耳是此临床综合征的发病部位。75年前，内淋巴积水被发现是梅尼埃病的病理特征。然而这一病理发现只能在尸检组织学研究中得到证实。由于诊断困难、各种听前庭症状的多种表现，单纯基于临床表现的诊断分类系统不断进行改良，并没有科学文献的发表中还没有统一使用。近来内耳高分辨核磁的发展使得我们能观察到可疑梅尼埃病人的内淋巴积水。在这次回顾中，我们总结了基于评估分析得到的可疑梅尼埃病的影像学知识。这表明内淋巴积水不仅是短暂性听力和前庭功能失调临床综合征发作的病因，同时会导致前庭和耳蜗梅尼埃病等其他临床表现。为了明确并简化诊断性分类，我们以临床和影像学特征为基础，提出了一个新的术语即―“耳积水性疾病”。

Menière’sdiseaseisachronicconditionwithaprevalenceof–per,andcharacterizedbyepisodicattacksofvertigo,fluctuatinghearingloss,tinnitus,auralpressureandaprogressivelossofaudiovestibularfunctions.Overyearsago,ProsperMenièrewasthefirsttorecognizetheinnerearasthesiteoflesionforthisclinicalsyndrome.Over75yearsago,endolymphatichydropswasdiscoveredasthepathologiccorrelateofMenière’sdisease.However,thispathologicfindingcouldbeascertainedonlyinpost-mortemhistologicstudies.Duetothisdiagnosticdilemmaandthevariablemanifestationofthevariousaudiovestibularsymptoms,diagnosticclassificationsystemsbasedsolelyonclinicalfindingshavebeenrepeatedlymodifiedandhavenotbeenuniformlyusedinscientificpublicationsonMenière’sdisease.RecentdevelopmentsofhighresolutionMRimagingoftheinnerearhavenowenabledustovisualizein-vivoendolymphatichydropsinpatientswithsuspectedMenière’sdisease.Inthisreview,wesummarizetheexistingknowledgeonimagingbasedevaluationofpatientswithsuspectedMenière’sdisease.Theseindicatethatendolymphatichydropsnotonlyisresponsibleforthefull-blownclinicaltriadofsimultaneousattacksofauditoryandvestibulardysfunction,butalsoforotherclinicalpresentationssuchas“vestibular”and“cochlearMenière’sdisease”.Asaconsequence,weproposeanewterminologyof“HydropicEarDisease”whichisbasedonbothclinicalandimagingcharacteristicsofthesedisordersinordertoclarifyandsimplifytheirdiagnosticclassification.
